Fugitive Heads are dropped by formidable foes called Headrotten, which are encountered during Headhunt events. These are considered boss-level enemies and can withstand significant damage. Fortunately, defeating them—either solo or alongside other players in the open world—will guarantee that you receive Fugitive Heads.

Additionally, players can locate Fugitive Head bosses while exploring Roothold Dungeons within Diablo 4 Season 7. These dungeons are relatively compact and straightforward to complete. However, access requires players to obtain Whispering Wood Sigils, which serve a similar purpose as Nightmare Sigils. To acquire these sigils, players should engage in various events, unlock Silent Chests in Headhunt areas, or find them in Whisper Caches acquired from the Tree of Whispers.

Different Occult Gems come with varying requirements; some necessitate specific combinations of Witch Powers, while others require enemies to be impacted by Hexes, Auras, or both. Players can equip multiple Occult Gems simultaneously, but they can only be assigned to rings and amulets. Furthermore, equipping identical versions of the same gem is not permitted.
